Active Case Management Service
The Active Case Management Service supports individuals to ensure care is planned and co-ordinated. This means less duplication and fewer unnecessary trips to the hospital or doctor, saving the individual time and effort.
The principle aim of Active Case Management is to effectively manage people with long-term chronic conditions who are very high intensity users of primary and secondary care health care services. More specifically, Active Case Management aims to:
- Reduce inappropriate hospital admissions and length of stay
- Reduce admissions into residential care
- Reduce inappropriate contacts with GP’s
- Co-ordinate care and support people to remain in their own homes and empower individuals to live independently
The Active Case Management Service is aligned to sectors within the borough of Bury and work closely with the GP’s and District Nursing teams within their sector to meet the strategic and local aims for Active Case Management.

The introduction of the Active Case Managers role is one of the mechanisms chosen by the Department of Health to improve the lives of people with long term conditions. Active Case Managers are seen as key to delivering personalised managed care needs to adults with complex care needs.
Who are Active Case Managers?
Experienced senior nurses who work closely with both patients and their carer’s and GP to plan and organise care. They act as ‘case manager’ – a single point of contact for care, support and/or advice.
Who is eligible to access our service?
Adults, (age 18 years and above) with long term conditions or a complex range of conditions may be referred to the Active Case Management service.

Referrals can be made to the service via Bury registered GP’s or as part of discharge planning from hospital.
