Community Cardiac Service
The Community Cardiac Service delivers a high quality, patient centred cardiac care encompassing cardiac rehabilitation and heart failure.
Following standards for Cardiac Rehabilitation and Heart Failure in the National Service Framework, Phase 2 of the 4 phases of the cardiac rehabilitation and chronic heart failure management services were created within Community Services Bury (CSB).
Phase 2 of cardiac rehabilitation occurs in the early post discharge period. The aim of the programme is to reduce the patient's risk of subsequent cardiac problems and promote their return to a full and normal life.
Managing heart failure can start at any point after diagnosis; it is a continuing assessment, support, education and medication titration service with most patient contact at home. There is a weekly clinic held in two locations on alternate weeks.
The team also provides regular training and education to health care professionals in CSB and to staff in residential and nursing homes.
Who is eligible to access our service?
Cardiac rehabilitation
- Patients on discharge from coronary care unit via a referral from a specialist practitioner.
- Patients registered with a Bury GP
We are currently identifying patients at high risk of developing cardiovascular disease to offer screening.
Heart failure
- Referrals from GPs, consultants, other Nurse Specialists.
- Patients can also self refer back in to the service after they have been discharged should their condition deteriorate.
Members of the public registered with a Bury GP can also contact us via telephone for advice.
How to access this service
Cardiac rehabilitation
- Discharge referral via a cardiology nurse from hospital.
- Patients can email, fax or telephone the office for any information
Bury GP's can refer directly into the service.
- Direct referral of patients from the Cardiac Support Group registered with a Bury GP.
- Consultant Cardiologists can refer from the Rapid Acess Chest Pain Clinic
Heart failure
- Bury registered patients that have left ventricular systolic dysfunction diagnosed by an echocardiogram.
- Referrals from Bury GPs, consultants, other Nurse Specialists
- Patients can also self refer back in to the service after they have been discharged should their condition deteriorate
