District Nursing Service

District nurses play a crucial role in the primary health care team. A district nurse will visit you at home if you are housebound or if there is a nursing reason why a home visit is appropriate.

As well as providing direct patient care, district nurses also have a teaching role, working with patients to enable them to care for themselves, or with family members teaching them how to give care to their relatives.

District nurses play a vital role in keeping hospital admissions and re-admissions to a minimum, ensuring that patients can return to their own homes as soon as possible.

A large proportion of the District Nursing Team are qualified to prescribe many products used in the delivery of nursing care.

The district nurse will discuss your needs and plan your nursing care with you and anyone who regularly helps to care for you with your consent.

The nursing records will remain in your home, you have the right to look at these records at any time. Within these records will be contact details of the district nurse who is responsible for your care.

The nurse will ask for information about yourself; this will ensure you receive the most appropriate care and treatment. It may be necessary at times to share this information with your GP or other professionals involved in your care in order to provide the best treatment.

In addition, the members of the district nursing team also:

  • Provide specialist advice and guidance to other primary health care professionals
  • Work in partnership with other service providers
  • Provide structured education programmes for primary health care professionals and community members
  • Provide education, development and assessment of pre and post-registration students
Who is eligible to access our service?

Anyone over the age of 16 years who is registered with a GP within Community Services Bury who requires nursing care.

How to access our service

All GP practices have a named District Nurse who can be contacted for referral purposes.

Referrals are also accepted from other health care professionals.
