Diabetes Specialist Nurses

The Diabetes Specialist Nurses (DSNs) provide a range of diabetes services, training and education and often work closely with the Diabetic Specialist Dietetian (DSD) team. They are part of the multi-disciplinary Community Diabetes Service. 

The DSNs undertake

  • Community home visits
  • Insulin Initiation or Byetta starts
  • Nurse led drop-in clinics
  • Carbohydrate counting programme
  • Preconception and post-partum clinics
  • Input in to the Pennine Acute Hosptials NHS Trust Ante Natal Clinic at Fairfield General Hospital
  • Patient education programmes
  • Other health care professional training and education

The DSNs provide training and education for patients, GPs, practice nurses, community nursing staff, other health care professionals and residential home carers.

Patient education

The DSNs deliver the diabetes X-PERT patient structured education programmes:

  • For people with type 2 diabetes
  • People with type 2 diabetes using Insulin
  • People starting insulin therapy with either type 1 and type 2 diabetes
  • One to one patient diabetes education sessions

Healthcare professional training and education is provided by the DSNs and includes:

  • A diabetes rolling programme  for community nursing staff & other healthcare professionals
  • A 2 day update session aimed at GPs and practice nurses
  • Bi-monthly training sessions for residential care home carers
  • Housebound Assessment Training for community nursing staff
  • Blood Glucose Meter and Hypo Box Training for all healthcare professionals

All people with diabetes and their carers. Other health care professionals requiring support or advice in diabetes for the benefit of patients in their care.
