Discharge Liaison Service
Admission to and discharge from hospital can be a distressing time for individuals and their families. In some circumstances additional help and support will be required. In these cases effective hospital discharge can only be achieved through efficient planning and good partnership working.
Discharge from hospital is not an isolated event. It has to be planned for at the earliest opportunity.
The Discharge Liaison Service is involved in developing and assisting in the comprehensive planning of hospital discharges. The Discharge Liaison Team assists in the management of delays to hospital discharge and provides support in hospital admission avoidance. The Team respond within an hour to referrals from the Accident and Emergency department and the Emergency Admissions ward at Fairfield to undertake assessment which may identify an appropriate alternative to an acute hospital admission.
The Team consist of five registered nurses who work in conjunction with other professionals and across health and social care agencies.
Based at Fairfield General Hospital, the team provides a communication link between the acute hospital, primary care trusts, adult care/social services, and the private and voluntary sectors.
The Discharge Liaison Team provides a service which aims:
- To prevent inappropriate emergency admission to and expedite early discharge from hospital
- To ensure the smooth, safe and timely transition of care from hospital
- To reduce the length of stay and prevent unnecessary delays to discharge
- To promote independence and choice
- To reduce the number of people admitted into long term residential care directly from hospital
- To reduce the risk of hospital acquired infection by keeping length of in-patient stay to a minimum
The Team works across all the adult medical, surgical and specialist wards and departments within Fairfield General Hospital.
Who is eligible to access our service?
The Discharge Liaison Team has a patient profile which includes all adults admitted to Fairfield General Hospital and those who may require Intermediate Care services from a primary care setting.
How to access our service
The Discharge Liaison Team receives and makes referrals from and to primary care services, secondary care services, adult care services, the private sector and voluntary agencies. These referrals can be nation wide.
