Adult Learning Disability Team

The Adult Learning Disability Team is a multi-disciplinary team providing highly specialist assessment and intervention to people with complex and challenging needs. We offer support to families, specialist Learning Disability care providers and mainstream healthcare services. We deliver our services in a wide range of settings based on individual need and preference.

What do we offer?
  • Health assessments and interventions
  • Strategic health facilitation to enable people to access mainstream health services
  • Specialist assessments in relation to health, communication, swallowing difficulties, epilepsy, activities of daily living and dementia
  • Health promotion activities for example Rebound and healthy living groups, functional assessments and intervention to manage complex behaviours
  • Training for staff groups

Adults with a moderate/severe learning disability who are registered with a Bury GP.
