Motor Neurone Disease Team
Motor Neurone Disease (MND) is a progressive disease that leads to wasting of muscles, causing increasing loss of mobility in the limbs, and difficulties with speech, swallowing and breathing. This multi-disciplinary team meets monthly and work closely together to plan and provide effective, high quality care taking in to account the physical, social and psychological impact of MND on the client and carers.
A referral gives access to the whole Motor Neurone Disease (MND) team. The individual may need only one or two professionals or the whole team. Clients are seen in their own home and a joint plan of care is agreed.
Links are maintained with the MND Association Regional Development Care Advisor and other health care professionals involved in care.
