Rapid Response Service

The Rapid Response Service provides urgent assessment in the community and care for patients who are ill in order to avoid unnecessary hospital admissions. Patients have access to a multi-disciplinary team that is made up of nurses, physiotherapists, occupational therapists and social workers.

Patients are usually admitted, after assessment in their own home, to a nursing home within Bury for a period of up to 2 weeks. During this time the team provides assessment, monitors care, provides equipment when necessary and plans ongoing care and support when required. The patient's GP usually maintains ongoing responsibility for any medical care that is required whilst the patient is under the care of the service.

Patients can be referred onwards to the Intermediate Managed Care Service for further rehabilitation if this is required. The Rapid Response Service also works closely with Social Services to ensure that patients receive ongoing social care provision where necessary. Referral to other health services is also made should intervention be required on dischsarge from the service.

Who is eligible to access our service?

Anyone over the age of 65 years who live in Bury and are registered with a Bury GP.

Patients must have an acute medical need that would result in a hospital/nursing level of care should the Rapid Response Team not become involved.

How to access our service

Patients are unable to refer themselves at the present time.

However, patients are either referred to the Rapid Response Service by their GP after medical assessment, Accident and Emergency or by the Discharge Liaison Nurses who are based in the hospital.
