One Year On

In January 2009 Community Services Bury was formally established as an ‘arms length provider organisation’ and this report seeks to provide an update on progress and achievements since our inception.

Who we are…

Community Services Bury provides a wide range of services across Bury, as part of Bury Primary Care Trust.

We work according to the standards and policies that cover all NHS organisations. We aim to deliver high quality services, which provide excellent patient experiences and represent value for money.

We work in partnership with a variety of NHS, Local Authority, private and voluntary organisations to provide integrated and seamless care. We continue to develop integrated ways of working with our partners to benefit the patients we serve.

Good news stories this year…

Organising ourselves more efficiently

  • Implemented Lorenzo™ Regional Care, the most advanced system from the National Programme for IT. Lorenzo™ Regional Care will enable better information sharing between services as the system is able to store scanned documents alongside the electronic patient record.
  • Further expansion of the Business Management Centre (BMC). The BMC has been developed as a focus for standardising referrals, information capture and work allocation to centrally manage referrals to Community Services Bury and facilitate choice for patients.
  • Actively seeking engagement and views of patients through a process called ‘Talkback’. Managers visit a range of services and get feedback directly from patients regarding their personal experiences.

More services in the community

Acute Care:

  • Dental Access Centre has extended its service provision to provide local access to minor oral surgery. The service is provided by a team of experienced clinicians and nurses located at Moorgate Primary Care Centre with state of the art facilities. This has resulted in patients receiving care closer to home in a more timely, accessible manner.
  • Introduced the Bury Urgent Care Service in partnership with a local GP practice and an Out of Hours service provider at Moorgate Primary Care Centre.

Planned Care:

  • Established a one stop clinic based at Radcliffe Primary Care Centre that offers an accurate diagnosis for patients with circulatory problems in their legs. This ensures they receive the correct treatment in the community to promote effective care; reducing repeated appointments and/or admission to hospital.

Staying Healthy:

  • The Community Nursing Service contributed to the local delivery of the Swine Flu programme in accordance with national direction in partnership with the Public Health Department.

Children’s Pathway:

  • Recruitment of a Paediatric Diabetes Specialist Nurse to provide a specialist resource within the Children’s Nursing Team and will offer an improved service for children and young people with diabetes in the community.
Our priorities for 2010/11determined by national requirements and local need:

During this year we will be focussing on making our services as efficient as possible and working with partners to provide seamless care within a framework of Quality, Innovation, Productivity and Prevention (QIPP).

Quality

  • Registration with the Care Quality Commission, the independent regulator of health and social care in England.
  • Introduction of Quality Indicators to help measure the quality of care clinicians deliver to ensure standards are maintained.

Innovation

  • Continue to work with local partners and patients to deliver “Care in Appropriate Setting Programme” with an emphasis on patient pathways providing care closer to home and improved outcomes for patients.
  •  Next phase of Lorenzo™ Regional Care will deliver mobile and wireless technology to enhance patient care and patient experience by enabling clinicians to access and record information in patients’ electronic records, within their homes.

Productivity

  • Implementing Productive Community Services - an organisation-wide change programme which engages front line teams in improving quality and productivity. Applying more efficient systems and processes to help us to work smarter to enhance the patient experience and allow frontline staff more time to spend on patient care.

Prevention

  • More focus on supporting people with Long Term Conditions, in partnership with them to live healthy and independent lives and prevent avoidable emergency admissions to hospital.
