Information available to patients
There is lots of information available about the NHS Care Records Service:
All patients who are registered at a participating GP Practice should have received an information pack.
The information pack now contains:
- Patient letter that is sent to every patient whose GP practice participates in Summary Care Records
- NHS Summary Care Leaflet explains how the NHS is changing the way it manages health records and gives an overview of Summary Care Records.
- An opt- out form
You can view a DVD about Summary Care Records here. This DVD is used in Bury schools to inform teenagers about Summary Care Records but provides a good introduction for people of all ages.
The Patient Advice and Liaison Service (PALS) is available at NHS Bury, 21 Silver Street, Bury, BL9 0EN. T: 0800 328 3166 Monday to Friday 9am-5pm. PALS are very experienced with Summary Care Records and will be more than happy to talk through your options and provide you with advice and support. PALS will treat all your information confidentially.
