25 GP Practices have created more than 137,000 Summary Care Records for their patients at the following practices:

In early 2012, patients registered at the seven remaining GP practices were sent a letter informing them of their options. Summary Care Records will be created for all patients that want them from April 2012. These remaining practices are:
